From the very start, we connect directly with the referring provider and care team to gather the patient’s history, medications, and current treatment plan. Using advanced diagnostic tools, we then assess the wound and determine if our specialized treatments, including skin substitutes and advanced therapies, are appropriate.
Step 2: Shared Treatment Plan
After our assessment, we create a customized wound care plan. This plan is reviewed with the patient, their family, and their PCP or facility provider. Every member of the care team knows exactly what treatment steps will be taken and what outcomes are expected. This eliminates confusion and ensures continuity of care.
Step 3: Ongoing Coordination and Communication
